Application for Change in Location for an Existing Video Gaming License

This application should only be submitted by a current Video Gaming Licensee whose establishment is moving to a new location. Applicants must already hold an active City of Bloomington Video Gaming License to qualify for this application type.

Required at Time of Submission
  • Computer Generated Site/Floor Plan -
    • A *floor plan, drawn to scale using a computer, detailing the overall layout of the establishment, including:
      • the location and count of dining seating;
      • the location and count of video gaming terminals with seating for said terminals;
      • other significant features of the establishment, including exit locations, restrooms, other rooms, equipment, etc.
*A high-resolution electronic copy of the floor plan shall be submitted. Hand-drawn floor plans and floor plans not including a scale will not be accepted.

Additional Requirements
For a Video Gaming License location change, the establishment must hold an active City and State of IL Liquor License and a State of IL Video Gaming License in order for a City of Bloomington Video Gaming License to be active at the new location. Once the State and City Liquor Licenses are active at the new location, Video Gaming can be active as long as the establishment has been approved for the Video Gaming Location Change. If approved, and the establishment holds the updated Video Gaming license with the State of IL, an electronic copy should be provided to the City Clerk Department to keep on file.

Review of Application
Review of the submitted Change of Location Video Gaming Application will begin following the outcome of the submitted Liquor License Application by the City Council.
